Referrals For Residential Programs

Inquiries and referrals may be made to the director of intake and marketing or the program director. An application and collateral information will be reviewed. When possible, a pre-placement interview with the youth and family will be scheduled on campus or in the youth's home community. When the youth is accepted, placement in the program occurs as soon as possible. A youth's placement at an LSS residential program is based on the program's ability to help the youth and family meet their needs.

When it is determined appropriate to continue the application process, please complete the application forms and mail or fax to the director of the residential center(s) at the address(s) provided on the application.

The referral provides information to determine if a child meets the criteria for care. Please submit a completed referral form below. An LSS representative will contact you as soon as possible to continue the application process.
