A child’s admission at any of the LSS residential facilities is based on the center's ability to help the child and family meet their needs. To make this determination a referral, application, and pre-placement interview must occur
Referral
The referral provides information to determine if a child meets the criteria
for care. Please submit a completed referral
form. An LSS representative
will contact you as soon as possible to continue the application process.
Application
When it is determined to continue the application process, please complete
the application
forms and mail or fax to the director of the residential
center(s) at the address(s) provided on the application.
Pre-Placement Interview
When the application and collateral information is received, a pre-placement
interview with the child and family is scheduled on campus or in the
child’s home community. If the child is accepted, placement occurs
as soon as possible.
