Part-time (28 hours a week), day time and some evening hours may be required depending on the needs of the client/family.
Assess client and his/her families’ needs, and assist them with appropriate services, support, decisions, and plans. Coordinate appropriate resources and community referrals. Provide follow-up assistance as needed. Prepare accurate and timely documentation and reports as necessary.
Bachelor’s degree required. Applicant must be at least 21 years of age, be able to complete orientation training according to agency program standards, and successfully complete required background checks. Candidate must possess a valid driver’s license and meet agency driving requirements.
Salary: Depends on experience and qualifications.
