Case Manager - internal applicants only

Summit Oaks Center, Residential Services for Children & Youth, Sioux Falls

Hours:
Full-time employment to include day, evening, some weekends and on-call

Responsibilities:
Work collaboratively with clinical therapists and direct care staff in relation to assigned caseload. Complete monthly and quarterly reports according to licensing standards. Maintain client files according to agency standards. Network with community resources and provide individual and group independent living skills training.

Communicate with placing workers, parents, school, and other collateral contacts regarding client progress. Complete paperwork related to program participation, family involvement, significant incidents, etc. Develop and facilitate groups. Provide leadership during emergency/emotional situations. Write or receive orders for emergency safety interventions on rotation.

Qualifications:
Bachelor’s degree required in social work or related field; plus 1 to 2 years related experience. Licensed Social Worker or Licensed Associate Social Worker through the SD Board of Social Work Examiners is preferred; or willing/able to complete such licensure. Must successfully complete all agency background checks. Requires valid driver’s license and ability to obtain approved driver status.

Salary: Dependent on experience and qualifications.
